Mission Statement
The mission of the H.O.P.E. Unit is to provide for the holistic, quality, safe and cost-effective delivery of services in a client/family-centered health care setting.
What the H.O.P.E. Unit Can Do For You:
The H.O.P.E. Unit provides health education and transportation services.
The H.O.P.E. Unit plans, develops, and implements health education programs consistent with the existing needs of the community. Our emphasis is health promotion, and disease prevention.

Health Education
The goal of the health educator is to plan and implement quality health education programs for the SNHS community. Major emphasis is placed on wellness and health promotion. Health education builds peoples' knowledge, skills, and positive attitudes about health. Health education teaches about physical, mental, emotional, spiritual, and social health, as well as sexual and reproductive health. It motivates people to improve and maintain their health, prevent disease, and reduce risky behaviors. Presentations are offered to individuals and small or large groups. Table displays have valuable information and incentives and are located in the lobby’s. Also visit us at community events such as HERO, Community Fair and the Fall Festival.
Medical Transportation
Priority is given to patients in need of dialysis or chemotherapy; and is provided once other resources have been determined ineligible. Patients who have Medicaid, will be required to apply for Medicaid transportation. Transportation is available to medical appointments at SNHS. 72 hours advance notice is required. It is the patient/client's responsibility to request transportation.
